Requirements

  1. Proof of Identity (i.e. Driver’s License, Valid Photo ID, etc.)
  2. Social Security card
  3. Driver’s License
    with a good driving record.
  4. Certification (+skills), Virginia License RN/HHA/LPN/PT/OT/SP
  5. CPR & First Aid
  6. TB (PPD) Screening Within six (6) months of application/Chest X-Ray (Valid for 5 Years)
  7. Physical Clearance
  8. Background Check
    For Virginia Residents you will need background checks conducted.
  9. HEP B Vaccine Records / Declination Form / QIDP / DSPs
